Health Coaches for Hypertension Control aims to improve hypertension self-management through small group educational sessions and support offered by trained Health Coaches. Participants will increase their knowledge about hypertension self-management, become more prepared to make lifestyle changes, and learn about how to make changes in the areas of nutrition, physical activity, stress management, tobacco use, and medication management. This workshop starts September 3 and ends October 22.
